          1. Introduction

1.1 PureLife Gloves, LLC, trading as PureLife Dental (“PureLife”, “we” or “us”) owns and operates the website (the “Site”), which is hosted in the United States.

1.2 We are the controller of personal data obtained via our Site, meaning we are the organization legally responsible for deciding how and for what purposes it is used.

1.3 We take your privacy very seriously. This Privacy Policy explains to you, as a visitor to our Site ("you" or "your"), how and why we collect, store, use and share any personal information relating to your use of our Site. This Privacy Policy also explains your rights in relation to your personal information and how to contact us.

1.4 All employees and those with whom we share personal information must adhere to this Privacy Policy.

          2. Information We Collect

2.1 We collect personal information about you to deliver the products and services requested and to help improve your experience on our Site.

2.2 We limit the amount of personal information collected to that required to support our specific purpose in requesting it. The following are some examples of the ways in which we may collect information from you and the purposes for which they are collected:

     2.1.1 Registration

               We collect personal information from those of you who register to order products and services from our Site. During registration, you will be asked to submit your contact information (such as valid email address, name, address, phone number and dental license number). You may also be required to verify your identity (such as date of birth, driver’s license or state identification number).

     2.1.2 Ordering

               We collect personal information (such as name, billing information, transaction and payment card or other payment method information and ship-to address) from you to facilitate the ordering process. We retain this information for any future orders, along with a record of the products and services you have previously purchased.

     2.1.3 Contact Forms

               We collect personal information from you (such as name, phone number and email) on the Site's contact forms. This allows us to respond to your request in a timely manner.

     2.1.4 Marketing-Related Email Updates

               We collect personal information (such as name and email address) from you when you choose to receive marketing-related email updates upon registration on the Site for an online account. You may elect to opt-out of receiving such emails by clearing the checkbox on the registration form and as provided below.

     2.1.5 Transaction-Related Email Updates

               We collect personal information (such as name and email address) from you when you choose to create an online account for transactional purposes such as order confirmation, order status, and other reminders. You may not opt-out of these email updates as they are necessary to ensure orders are properly communicated and tracked.

     2.1.6 Use of Cookies and Web Beacons

               We use cookies to track Site usage and trends, to improve the quality of the Site, and to customize your experience on the Site. We save a "cookie" – a small text file – on your computer. You can remove or block cookies using the settings in your browser, but in some cases doing so may impact your ability to use the Site.

               We use one type of cookie, called a "persistent" cookie, once you have signed in to your account to identify you as a returning visitor to the Site. We use another type of cookie, called a "session" cookie, to identify a particular visit to the Site. Session cookies used by the Site expire after you close your web browser.

               We may use web "beacons" to allow us to know if you visited a certain page, opened an email from us or to determine if ad banners on our Site were effective.

               Some content or applications, including advertisements, on the Site are served by third parties, including advertisers, ad networks and servers, content providers and application providers. These third parties may use cookies alone or in conjunction with web beacons or other tracking technologies to collect information about you when you use our Site. The information they collect may be associated with your personal information or they may collect information, including personal information, about your online activities over time and across different Sites and other online services.                We do not control these third parties' tracking technologies or how they may be used. If you have any questions about an advertisement or other targeted content, you should contact the responsible provider directly.

     2.1.8 Log Files

               When you visit the Site, we collect the Internet Protocol (“IP”) address connected to your computer (or the proxy server you use to access the World Wide Web), your computer operating system, the type of browser you are using, mobile device operating system (if you are accessing the Site using a mobile device), as well as the name of your Internet Service Provider (“ISP”) or your mobile carrier. We use this information to analyze overall trends to help improve the Site. We do not share with third parties, the linkage between your IP address and your information on the Site, unless required by law to do so.

          3. How We Use Your Information

3.1 We use your information to offer and provide you with products and services and to support our core business functions. These include order or service fulfilment, internal business processes, marketing, authentication, fraud prevention and other correlated functions.

     Some examples include:

     3.1.1 to fulfil your requests for products and services and communicate with you about those requests;

     3.1.2 to register and service your account;

     3.1.3 to store your preferences and settings;

     3.1.4 to provide customer service and to alert you to product information, including recalls;

     3.1.5 to help us improve and customize our service offerings, the Site and marketing;

     3.1.6 to send you information about our products, services and promotions; and

     3.1.7 to protect the security or integrity of this Site and our business.

3.2 To accomplish these purposes, we may combine personal and non-personal information we collect online with offline information, including information from third parties. We may transfer or disclose your information with our affiliated companies for these purposes.

3.4 As permitted by applicable law, we do not treat deidentified data or aggregate customer information as personal information and we reserve the right to convert, or permit others to convert, your personal information into deidentified data or aggregate consumer information.

          4. Ways We Share Your Information Outside PureLife

4.1 We may share your personal information with any subsidiaries, affiliates, licensees, service providers, suppliers, vendors, distributors, assigns, successors, or related entities and their respective owners. In some cases, we may sell or rent your personal information, including but not limited to our receipt of a rebate upon a product purchase. We will not share your personal information with third parties to directly market their products to you who are not part of our business operations or that are not within our supply chain of products and services.

4.2 We may disclose your personal information as follows:

     4.2.1 Service Providers, Suppliers & Vendors

               We may disclose or otherwise make information about you available to third parties that help with our business operations. Examples are shipping vendors, billing and refund vendors, payment card processors, professional advisors and companies that help us improve our product and service offerings on our Site. Your information may be stored and processed by our service providers or suppliers in the United States or other locations where the service providers or suppliers maintain facilities. We require our service providers and suppliers to keep your information secure. In addition, our service providers and suppliers may not use or disclose personal information for any purpose other than providing the services or supplies on our behalf.

     4.2.2 Legal Requirements and Protection of PureLife and Others

               We may need to disclose your personal information and/or information about your activities as a user of the Site when required by subpoena or other legal process, or if we have a good faith belief that disclosure is necessary to:

               (a) investigate, prevent or take action regarding suspected or actual illegal activities or to assist government enforcement agencies;

               (b) enforce this Privacy Policy, investigate and defend ourselves against any third-party claims or allegations, or protect the security or integrity of our Site; and/or

               (c) exercise or protect the rights, property or personal safety of PureLife, our users, employees, agents or others.

     4.2.3 Business Transfers

               We may also disclose your information to a third party as part of reorganization or a sale involving all or a portion of PureLife, or its subsidiaries or divisions. Any third party to which we transfer or sell Site related assets will have the right to continue to use the personal and other information that you provide to us.

               We may, without restriction, disclose aggregated and anonymized information about our users and information that does not identify any individual or disclose personal information.

          5. Opting-Out of Email Communications

You may opt-out of PureLife email marketing communications by clicking the "unsubscribe" link in the emails you receive or by sending an email to [email protected] with the subject line, "UNSUBSCRIBE". Please be aware that, even if you have opted-out of receiving marketing communications from us, we may still contact you for transactional purposes, such as customer service, product information, service or reminder notices, or recalls. We may also need to contact you via email with questions or information regarding your order, such as order status, shipment or refunds.

          6. Other Websites

Our Site may contain links to other websites on the Internet that are owned and operated by third parties. This Privacy Policy does not cover the information practices of those websites linked to our Site. We encourage you to be aware when you leave our Site and to read the privacy policies and statements of each website that may collect your personal information.

          7. Children and Minors

Our services are not directed to children. If a parent or guardian becomes aware that his or her child has provided us with personal information without their consent, please contact us. If we become aware that a child has registered for a service and has provided us with personal information, we will delete such information from our files.

          8. Information Security

8.1 We are committed to security, confidentiality and integrity of personal information in accordance with legal requirements. We take commercially reasonable precautions to keep personal information secure against unauthorized access and use and we periodically review our security measures. We are committed to processing your data in a secure manner and have put in place specific technical and organizational measures to prevent the personal information we hold from being accidentally or deliberately compromised.

8.2 We utilize current industry accepted encryption technologies and strengths to reduce the risk that others are able to view information passing between our web servers and your browser. Please be aware that these protection tools do not protect information that is not collected through our Site, such as information provided to us by e-mail.

8.3 We implement administrative, technical and physical safeguards to protect the content you post on our servers. However, since the Internet is not a completely secure environment, we cannot ensure or warrant the security of any information you transmit to us. There are no guarantees that information cannot or will not be accessed, disclosed, altered or destroyed by breach of any of our administrative, technical or physical controls. It is your responsibility to protect the security of your customer log-in information (your username and password).

8.4 We also conduct information risk assessments, we train our staff to understand the importance of protecting personal information, and we are responsibly managing access rights within the company. We include both physical security and IT security in our overall data security approach. We are diligent in selecting service providers that process personal information on our behalf so that they also ensure appropriate technical and organizational measures to protect the data.

8.5 We make reasonable efforts to notify individuals and regulatory authorities, as required by law, if we reasonably believe that personal information has been stolen, disclosed, altered or infringed by an unauthorized person.

          9. Your Data Protection Rights

9.1 If you resident of a jurisdiction with data protection laws, we are committed to supporting your rights granted by such applicable data protection laws.

9.2 Under certain circumstances, you may have the privacy rights described in clause 7 below.

9.3 Any request you submit to us is subject to an identification and verification process. We will not fulfil your request unless you have provided sufficient information for us to reasonably verify you are the individual about whom we collected personal information.

9.4 If we cannot comply with a request, we will explain the reasons in our response. We will use personal information provided in a verifiable request only to verify your identity or authority to make the request and to track and document request responses.

9.5 We will make commercially reasonable efforts to identify personal information that we collect, process, store, disclose and otherwise use and to respond to your applicable privacy rights requests. In some cases, we may suggest that you receive the most recent or a summary of your personal information and give you the opportunity to elect whether you want the rest.

9.6 We will typically not charge a fee to fully respond to your requests; provided, however, that we may charge a reasonable fee, or refuse to act upon a request, if your request is excessive, repetitive, unfounded or overly burdensome. If we determine that the request warrants a fee, or that we may refuse it, we will give you notice explaining why we made that decision. You will be provided a cost estimate and the opportunity to accept such fees before we will charge you for responding to your request.

9.7 Pursuant to applicable data protection laws, your privacy rights may include the following:

     9.7.1 Information Rights

               You may have the right to send us a request, no more than twice in a twelve-month period, for any of the following for the period that is 12 months prior to the request date:

               (a) the categories of personal information we have collected about you;

               (b) the categories of sources from which we collected your personal information;

               (c) the business or commercial purposes for our collecting or selling your personal information;

               (d) the categories of third parties to whom we have shared your personal information;

               (e) the specific pieces of personal information we have collected about you;

               (f) a list of the categories of personal information disclosed for a business purpose in the prior 12 months, or that no disclosure occurred; or

               (g) a list of the categories of personal information sold about you in the prior 12 months, or that no sale occurred. If we sold your personal information, we will explain the categories of your personal information we have sold and the categories of third parties to which we sold personal information, by categories of personal information sold for each third party.

     9.7.2 Correction

               You may have the right to ask us to make corrections to inaccurate or incomplete personal information about you.

     9.7.3 Erasure

               You may have the right to request the deletion of your personal information we have collected from you, subject to certain conditions and limitations under the law.

     9.7.4 Restriction of Use

               You may have the right to request the restriction of processing of personal information about you.

     9.7.5 Opt-Out of the Sale of Personal Information

               To the extent we sell your personal information as the term “sell” is defined under the California Consumer Privacy Act, you have the right to opt-out of the sale of your personal information by us to third parties at any time.

We reserve the right to modify this Privacy Policy and related business practices at any time. We will duly inform you of any changes in the posted Privacy Policy.

The Privacy Policy will indicate the last date it was revised.

Compliance with the Sunshine Act

The Open Payments/Physician Payments Sunshine Act (“Sunshine Act”) was signed into law as part of the Affordable Care Act in March 2010

The Sunshine Act requires certain healthcare companies, including PureLife, to report to the federal government any payments or transfers of value made to “physicians” (such term defined to include dentists), with certain exceptions, such as discounts and rebates provided in the ordinary course of business. The Sunshine Act also requires companies to report any physician ownership or investment interests.

PureLife is committed to ensuring that the data we submit under the Sunshine Act is as accurate as possible and has robust processes in place to enable tracking and reporting in compliance with its obligations.
